The Palliative Care Connect Directory is a database of palliative care and bereavement services and resources. The Directory is designed to be an easily accessible platform of current information about health and support services in South Australia. It can be searched by postcode or by type of service. The directory can be used by both community members looking for a service and service providers looking to refer or provide direction to patients and clients. You can also suggest additional resources that you think could be helpful for others in the SA community.
